ObjectiveOverweight and obesity [body mass index ≥ 25 kg/m2] are associated with poorer prognosis among women with breast cancer, and weight gain is common during treatment. Symptoms of depression and anxiety are also highly prevalent in women with breast cancer and may be exacerbated by post-diagnosis weight gain. Altered brain function may underlie psychological distress. Thus, this secondary analysis examined the relationship between BMI, psychological health, and resting state functional connectivity among women with breast cancer.MethodsThe sample included 34 post-menopausal women (...) newly diagnosed with Stage 0-IIa breast cancer who were enrolled in a 6-month randomized controlled trial of aerobic exercise vs. usual care. At baseline prior to randomization, whole-brain analyses were conducted to evaluate the relationship between BMI and seed-to-voxel rsFC of the hippocampus and amygdala. Connectivity values from significant clusters were then extracted and examined as predictors of self-reported depression and anxiety.ResultsMean BMI was in the obese range. For both seeds examined, higher BMI was associated with lower rsFC with regions of prefrontal cortex, including ventrolateral PFC, dorsolateral PFC, and superior frontal gyrus. Hippocampal connectivity with the vlPFC was negatively correlated with self-reported anxiety.ConclusionHigher BMI was associated with lower hippocampal and amygdala connectivity to regions of PFC implicated in cognitive control and emotion regulation. BMI-related differences in hippocampal and amygdala connectivity following a recent breast cancer diagnosis may relate to future worsening of psychological functioning during treatment and remission. Additional longitudinal research exploring this hypothesis is warranted. (shrink)

Scholars have debated whether the sentiment of sixteenth century reformers against material forms of worship derived from certain Neo-Platonic ideas proliferating in parts of Europe and disseminated by Erasmus or from strictly Scriptural principles that were initially formulated by the Old Testament prophets and given fuller expression in the New. This essay studies the reformers′ interpretation of the phrase logikē latreia at Romans 12:1, as well as other key passages. It concludes that, whether consciously or subconsciously, the reformers borrowed language (...) concerning the material-spiritual dichotomy of worship, not directly from Neo-Platonists, but from a commonplace used by numerous Roman writers. Early Church Fathers had long ago turned the same commonplace against pagan rivals, but now the reformers were employing it against the Catholic Church itself. (shrink)

In my comments, I address two issues that are important but not central to the paper under review here. First, I present a reading of the postphenomenological concept of multistability by going back to Merleau-Ponty’s notion of the primacy of perception. I conclude that assertions affirming the multistability of technologies should not be seen as merely empirical. Second, I address the adequacy of using the language of ‘empirical’ and ‘transcendental’ as a means to categorize exclusionary approaches in philosophy of technology.

The book explores a previously unstudied work of Theodore Beza, the Cato Censorius Christianus (1591). When read in conjunction with the works and correspondence of Beza and his colleagues (Simon Goulart, Lambert Daneau, Peter Martyr Vermigli, among others), the poems of the Cato reveal the theoretical underpinnings of the disciplinary activity during the period. This innovative book is a practical guide to using the therapy, covering each disorder individually. Particular attention is paid to overcoming problems encountered during treatment.

Background 300 million operations and procedures are performed annually across the world, all of which require a patient’s informed consent. No standardised measure of the consent process exists in current clinical practice. We aimed to define a core outcome set for informed consent for therapy. Methods The core outcome set was developed in accordance with a predefined research protocol and the Core OutcoMes in Effectiveness Trials methodology comprising systematic review, qualitative semi structured interviews, a modified Delphi process and consensus webinars (...) to ratify outcomes for inclusion in the final core outcome set.. Participants from all key stakeholder groups took part in the process, including patients and the public, healthcare practitioners and consent researchers. Results 36 outcome domains were synthesised through systematic review and organised into a consent taxonomy. 41 semi-structured interviews were performed with all consent stakeholders groups. 164 participants from all stakeholder groups across 8 countries completed Delphi Round 1 and 125 completed Round 2. 11 outcomes met the ‘consensus in’ criteria. 6 met ‘consensus in’ all stakeholder groups and were included directly in the final core outcome set. 5 remaining outcomes meeting ‘consensus in’ were ratified over two consensus webinars. 9 core outcomes were included in the final core outcome set: Satisfaction with the quality and amount of information, Patient feeling that there was a choice, Patient feeling that the decision to consent was their own, Confidence in the decision made, Satisfaction with communication, Trust in the clinician, Patient satisfaction with the consent process, Patient rated adequacy of time and opportunity to ask questions. Conclusion This international mixed-methods qualitative study is the first of its kind to define a core outcome set for informed consent for intervention. It defines what outcomes are of importance to key stakeholders in the consent process and is a forward step towards standardising future consent research. (shrink)

Background Informed consent is an integral component of good medical practice. Many researchers have investigated measures to improve the quality of informed consent, but it is not clear which techniques work best and why. To address this problem, we propose developing a core outcome set to evaluate interventions designed to improve the consent process for surgery in adult patients with capacity. Part of this process involves reviewing existing research that has reported what is important to patients and doctors in the (...) informed consent process. Methods This qualitative synthesis comprises four phases: identification of published papers and determining their relevance; appraisal of the quality of the papers; identification and summary of the key findings from each paper while determining the definitiveness of each finding against the primary data; comparison of key themes between papers such that findings are linked across studies. Results Searches of bibliographic databases returned 11,073 titles. Of these, 16 studies met the inclusion criteria. Studies were published between 1996 and 2016 and included a total of 367 patients and 74 health care providers. Thirteen studies collected data using in-depth interviews and constant comparison was the most common means of qualitative analysis. A total of 94 findings were extracted from the primary papers and divided into 17 categories and ultimately 6 synthesised findings related to: patient characteristics, knowledge, communication, the model patient, trust and decision making. Conclusions This qualitative meta-aggregation is the first to examine the issue of informed consent for surgery. It has revealed several outcomes deemed important to capture by patients and clinicians when evaluating the quality of a consent process. Some of these outcomes have not been examined previously in research comparing methods for informed consent. This review is an important step in the development of a COS to evaluate interventions designed to improve the consent process for surgery. Increasing behavioural evidence suggests that expert video game players (VGPs) show enhanced visual attention and visuospatial abilities, but what underlies these enhancements remains unclear. We administered the Poffenberger paradigm with concurrent electroencephalogram (EEG) recording to assess occipital N1 latencies and interhemispheric transfer time (IHTT) in expert VGPs. Participants comprised 15 right-handed male expert VGPs and 16 non-VGP controls matched for age, handedness, IQ and years of education. Expert VGPs began playing before age 10, had a minimum 8 years experience, and (...) maintained playtime of at least 20 hours per week over the last 6 months. Non-VGPs had little-to-no game play experience (maximum 1.5 years). Participants responded to checkerboard stimuli presented to the left and right visual fields while 128-channel EEG was recorded. Expert VGPs responded significantly more quickly than non-VGPs. Expert VGPs also had significantly earlier occipital N1s in direct visual pathways (the hemisphere contralateral to the visual field in which the stimulus was presented). IHTT was calculated by comparing the latencies of occipital N1 components between hemispheres. No significant between-group differences in electrophysiological estimates of IHTT were found. Shorter N1 latencies may enable expert VGPs to discriminate attended visual stimuli significantly earlier than non-VGPs and contribute to faster responding in visual tasks. As successful video-game play requires precise, time pressured, bimanual motor movements in response to complex visual stimuli, which in this sample began during early childhood, these differences may reflect the experience and training involved during the development of video-game expertise, but training studies are needed to test this prediction. In The Axiological Status of Theism and Other Worldviews (2020), I defend the Complete Understanding Argument for anti-theism, which says that God’s existence makes the world worse with respect to our ability to understand it. In a recent article, Roberto Di Ceglie offers three objections to my argument. I seek to rescue my argument by showing (1) that understanding can come in degrees; (2) that I’m not a consequentialist about the value of understanding; and (3) that my argument is consistent (...) with God providing us with sufficient knowledge of important spiritual matters. Di Ceglie’s objections point to future areas for fruitful exploration but do not defeat my argument. (shrink)

R kirk ("analysis", volume 33, 1973, pages 195-201) proposes an argument against quine's deduction of indeterminacy of translation from underdetermination of physical theory. the present paper is a reply to kirk, aimed primarily at showing that his argument is "ignoratio elenchi".

In my recent book, The Epistemic Benefits of Disagreement, I develop a defense of non-conciliationism, but one that only applies in research contexts: Epistemic benefits are more likely in the offing if inquirers stick to their guns in the face of disagreement. I aim to expand my original account by examining its implications for non-inquiry beliefs. I’m particularly interested in broader worldview disagreements. I want to examine how inquirers should react upon discovering that they disagree about the truth value of (...) a particular proposition because they disagree about a whole host of related propositions. I argue that in many ways, worldview disagreements are easier to work with than disagreement over isolated propositions, in part because it is easier to provide a set of criteria by which to evaluate worldviews. I conclude that my original argument can, at least in part, be successfully expanded to include worldview disagreement. (shrink)

Identifying the determinants of reproductive success in small-scale societies is critical for understanding how natural selection has shaped human evolution and behavior. The available evidence suggests that status-accruing behaviors such as hunting and prosociality are pathways to reproductive success, but social egalitarianism may diminish this pathway. Here we introduce a mixed longitudinal/cross-sectional dataset based on 45 years of research with the Batek, a population of egalitarian rain forest hunter-gatherers in Peninsular Malaysia, and use it to test the effects of four (...) predictors of lifetime reproductive success: foraging return rate, sharing proclivity, cooperative foraging tendency, and kin presence. We found that none of these factors can explain variation in lifetime reproduction among males or females. We suggest that social egalitarianism, combined with strikingly low infant and juvenile mortality rates, can mediate the pathway between foraging, status-accruing behavior, and reproductive success. Our approach advocates for greater theoretical and empirical attention to quantitative social network measures, female foraging, and fitness outcomes. (shrink)

J. Andrew Kirk’s book examines – and eventually agrees with – Samuel Huntington’s controversial claim that a dangerous ‘Clash of Civilisations’ today poisons the Western-and-Islamic relationship. Kirk then suggests ways for improving these relations, highlighting Islam’s need to reform itself through a re-embrace of its more eirenic, prophetic Meccan phase. While there is much that is admirable here, the review below suggests Kirk’s interpretation of the ‘prophetic’ relationship to government is too one-sidedly Anabaptist.

This paper re-examines an argument of kirk's aimed at refuting quine's inference from the underdetermination of physical theory to the indeterminacy of translation. it is claimed that kirk's argument is unsuccessful; unsuccessful, at any rate, if we make what has seemed until recently the only possible assumption about quine's criterion for individuating theories. but in recent publications quine has proposed a rather different criterion, and in the light of this, it is conceded, kirk's argument may well take (...) effect. it is then argued that quine's new criterion has disastrous effects; it not merely opens a way for kirk's criticism of one of the props of the indeterminacy thesis, but puts that thesis itself at risk. Rooted in a desire to conduct ethical research with Indigenous People rather than on them, this autoethnographic document-based case study relies on anti-colonial praxis and places the Navajo and non-Navajo perspectives of two researchers navigating the Navajo Nation Human Research Review Board’s (NNHRRB) processes for conducting human subjects research within Kirkness and Barnhardt’s (1991) “Four Rs” framework of respect, relevance, reciprocity, and responsibility. This case analyzes and presents thematic assertions drawn from entries of both authors’ reflective journals they kept while (...) working through the NNHRRB’s process for conducting research. While outcomes for children with T-cell acute lymphoblastic leukemia have improved dramatically, survival rates for patients with relapsed/refractory disease remain dismal. Prior studies indicate that glucocorticoid resistance is more common than resistance to other chemotherapies at relapse. In addition, failure to clear peripheral blasts during a prednisone prophase correlates with an elevated risk of relapse in newly diagnosed patients. Here we show that intrinsic GC resistance is present at diagnosis in early thymic precursor T-ALLs as well as in a subset (...) of non-ETP T-ALLs. GC-resistant non-ETP T-ALLs are characterized by strong induction of JAK/STAT signaling in response to interleukin-7 stimulation. Removing IL7 or inhibiting JAK/STAT signaling sensitizes these T-ALLs, and a subset of ETP T-ALLs, to GCs. The combination of the GC dexamethasone and the JAK1/2 inhibitor ruxolitinib altered the balance between pro- and anti-apoptotic factors in samples with IL7-dependent GC resistance, but not in samples with IL7-independent GC resistance. Together, these data suggest that the addition of ruxolitinib or other inhibitors of IL7 receptor/JAK/STAT signaling may enhance the efficacy of GCs in a biologically defined subset of T-ALL.Leukemia advance online publication, 30 May 2017; doi:10.1038/leu.2017.136. Positive changes or transformations have been the subject of study within spiritual traditions as well as humanistic and transpersonal psychology. The aim of the current study is to understand the process of transformation among Moslems in Indonesia, who follow spiritual practices, to achieve the nafs al-muṭma ínnah [tranquil self]. Ten participants in Yogyakarta province were involved in this study. They were recruited using nafs al-muṭmaʾinnah scale developed by the authors. In-depth interviews of both the participants and their significant others were (...) conducted. To analyse the data, an interpretative phenomenological analysis was employed. Five themes emerged from the data, including deviation from the basis of religious environment, spiritual discipline, transformative experiences, new characters and a continuous process. We conclude that participants experience spiritual-religious transformation and achieve the state of nafs al-muṭma ínnah [tranquil self]. The process of transformation occurs gradually after a sudden experience, and it continuously happens throughout the course of life. Three factors contribute to the transformation, including the role of predisposition, the role of a spiritual guide and the role of spiritual experiences.Contribution: As the study of spiritual-religious transformation is mainly conducted within a Judea-Christian tradition, this study significantly contributes to the literature by providing spiritual-religious transformation from an Islamic-Sufi perspective. DNA double‐strand breaks (DSBs) are extremely hazardous lesions for all DNA‐bearing organisms and the mechanisms of DSB repair are highly conserved. In the eukaryotic mitotic cell cycle, DSBs are often present following DNA replication while, in meiosis, hundreds of DSBs are generated as a prelude to the reshuffling of the maternally and paternally derived genomes. In both cases, the DSBs are repaired by a process called homologous recombinational repair (HRR), which utilises an intact DNA molecule as the repair template. Mitotic (...) and meiotic HRR are managed by ‘checkpoints’ that inhibit cell division until DSB repair is complete. Here we attempt to summarise the substantial recent progress in understanding the checkpoint management of HRR in mitosis (focussing mainly on mammals) and then go on to use this information as a framework for understanding the presumed checkpoint management of HRR in mammalian meiosis.
